This is a reminder that the submission deadline for the National Award =
for
Library Services is February 15, 2005.   Public or private nonprofit
institutions are eligible to receive this award. Nominated institutions =
must
be open to the general public for at least 120 days per year and be =
located
in the United States, the Commonwealth of Puerto Rico, the Virgin =
Islands,
Guam, American Samoa, the Commonwealth of Northern Mariana Islands, or =
the
Freely Associated States in the Pacific. Federally operated =
institutions are
not eligible for this award.

The National Award for Library Service honors outstanding libraries =
that
make significant and exceptional contributions to their communities.  =
They
are libraries that demonstrate extraordinary and innovative approaches =
to
public service, reaching beyond the expected levels of community =
outreach
and core programs generally associated with library services.

All applications must include the following information:=20

=20

*         Name of institution, institution's address, phone number, fax
number, website address; institution's Director/CEO, their email =
address,
phone number; type of Library (public, research, school, etc.);
institution's annual budget; the number of full and part-time staff; =
name of
nominating individual, their title, relationship to the institution, =
phone
number and email address. Applicants must answer the following =
questions:

*         Define the community the library is reaching through this
programming (not to exceed =BD  page)

*         What is the institution's goal for community service, and =
what is
the institution's philosophy or strategy for reaching this goal? (not =
to
exceed =BD page)

*         Describe the components of the community that work together =
with
the institution in developing and implementing its programming  ( not =
to
exceed =BD  page)

*         What is the library doing to ensure the ongoing nature of its
service to the community? Not to exceed =BD  page)

*         Describe recent examples of the institution's community =
service.
Achievements that might be highlighted include programming that =
demonstrates
how the institution has attracted new audiences, innovative programming =
that
addresses current educational, social, economic, or environmental =
issues;
and positive effects of the institution's collaboration with other =
public
organizations in the community. (not to exceed 3 pages)

Please submit no more than three letters of recommendation from
representatives of the community served by the library.  Send them with =
your
application or separately to IMLS.=20

products. If sent, these materials will not be returned.  Applications =
