[OPLINLIST] Online library card registration

I'm interested in hearing from libraries who offer online library card
registration. I see this was asked about on the list a couple of years
ago, but I didn't see any on-list responses.

I'd love to see examples of online registration forms. I'm also
interested in the procedure:

- Do you mail a card, or do they have to come into the library with
additional paperwork?
- Does anyone offer an "online only" library card--one that works to
access online materials but not physical materials? If so, how is that
policy different?
