I thought this message from Lorrie Germann (Lorrie.Germann at eTech.ohio.gov), State E-Rate Coordinator, would be helpful to you. Remember Form 486 is the eRate Form to indicate that your services have started. It can be filed online here: http://www.sl.universalservice.org/menu.asp


Please be aware of the upcoming April Form 486 Deadlines:

The deadline for filing a Form 486 is 120 days from the Service Start Date (typically July 1st of the funding year for recurring services) or from the issuance date of the Funding Commitment Decision Letter ("FCDL"), whichever is later.  At this point in the FY 2012 funding cycle, the Form 486 deadline is normally determined by the FCDL date.  On this basis, the Form 486 deadlines for the month of April 2013 are as follows:

If the Form 486 deadline is missed USAC will issue a letter notifying the applicant, and provide a 20-day grace period to submit the Form 486 without penalty.

If the extended Form 486 deadline is missed, USAC will adjust the Service Start Date of the FRNs on the FCC Form 486 to the date 120 days before the FCC Form 486 was received or postmarked. USAC will not pay discounts on services received before this date, and the applicant's funding commitment may be reduced.


Also, after you have submitted and certified an FCC Form 471 and USAC has determined that the form is in-window, USAC will issue you a Receipt Acknowledgment Letter (RAL)<http://www.usac.org/_res/documents/sl/pdf/samples/form-471RAL-applicant.pdf>. This letter contains a summary of the data you reported on the FCC Form 471 and gives you an opportunity to submit corrections that are the result of ministerial and clerical errors<http://www.usac.org/sl/applicants/step02/clerical-errors.aspx>.

If you find that information on your RAL is incorrect and/or missing, mark up a copy of your RAL as indicated in the instructions and submit your corrections to USAC.
