[OPLINLIST] Example of transportation legal form?

Hello,

Our library system has five small county branches that cover a large rural geographic area. At the end of the summer we always have a party in our city park to celebrate the end of our summer reading program where we hand out prizes, serve hot dogs and then the kids who finished get to swim in a private party in the city pool. This year we are trying something new and have decided to charter some small buses and vans from our local Goodwill to travel to each of our county branches and offer transportation to and from the party for kids who completed the program but do not have the means to travel into town.

We are requiring that each child must be accompanied by a responsible adult and would like to put together a form for that adult to sign stating that they alone are responsible for the safety and supervision of the child for the evening. So my questions are- has anyone else out there done something similar to this and have any advice or tips to share on how to make this run smoothly, and does anyone have an example of a similar form they have used in the past that they would be willing to share so we could take a look at the language? Thanks for any and all help!
