Important survey to identify libraries directly serving patrons with Alzheimer's and related dementias.

The Alzheimer's & Related Dementias Interest Group (IGARD) is in the process of creating guidelines for library service directly to persons diagnosed with dementia. As part of the due diligence process, we are seeking input from libraries across the country so that we can document already existing best practices.
If your library is currently offering, or has offered in the past, services and/or programming directly to members of your community diagnosed with dementia, please self-identify by completing this brief survey -  https://www.surveymonkey.com/s/services_dementia.

IGARD is an Interest Group of the Association of Specialized and Cooperative Library Agencies (ASCLA), which is a division of the American Library Association (ALA) - http://www.ala.org/ascla/interestgroups/igard.
