[OPLINLIST] Reading Tutors

We want to provide volunteer reading tutors for youth in our library, and
are still in the planning stages.   We do have someone to do background
checks on our volunteers.  Now we are thinking about permission slips for
the parents to release the library (and tutors) of liability (in case there
reading does not improve?)
Has anyone done this successfully in their library? Do you have a sample of
permission slips or any other suggestions for success?  Thanks for your
help!
