[OPLINLIST] Health Insurance Contribution

Hello,

I know this is asked on the annual public library statistics, but I don't see it in the report. I'm hoping to find some information on how libraries of comparable size handle their health insurance contribution.

If you are a library that has a budget of around $1 to $1.5 million, I'd appreciate knowing the following information:


1.       If you provide health insurance for your full time staff

2.       What percentage does the library pay vs. the percentage the staff member pays for the single plan.

3.       And, if it is easy to determine, some idea of what your monthly costs are for a single plan.  I understand now that some plans might have a different cost based on each employees age. I'm mainly looking to see anyone's pricing if you get one aggregate, use-based premium for your plan.

If I am missing where this is on the spreadsheet of the state report, please let me know. I feel stupid that I can't find it...
