We have had at least 3 in the past five years, Friday nights after close from 5:30 to 8:30, so, not too late.

We have a very simple form for parental permission.

You have to pick a night that does not conflict with other activities, especially if you are in a busy busy community. The last one we had was packed (about double what we anticipated) because we got a large number of last minute registrants. Someone texted the event to their friends omitting all the "get permission, sign up by XX date" so there were signups as parents dropped them off. So we ordered more pizza. Next time we will have a cutoff.

We usually staff the building with 4 people.

We kept them busy with organized (minute to win it, scavenger hunt) and random activities/games, better to not spend too much effort on the former lest they get bored. At least lay out the ground rules off the top.

They often come up with their own ideas, so if you have a teen advisory board or anime club the same kids will show. Last year a spontaneous hide and seek game erupted (and a few shelves fell down, but nobody was hurt), so roll with it if it fits your general guidelines, which they appreciate being relaxed for one evening a year.
