Item 21 attachments must also be submitted by the close of the window. <o:p></o:p></p><p class=MsoNormal style='text-align:justify'><o:p> </o:p></p><p class=MsoNormal style='text-align:justify'>Please be aware that a number of applicants have experienced problems with the electronic certification of their Form 471. The issue seems to be due to the PIN including a symbol sequence such as “&#”. If the system will not accept your PIN, be prepared to provide a paper certification. We have reported the issue to USAC, but it may not be resolved before the close of the window.<o:p></o:p></p><p class=MsoNormal style='text-align:justify'><o:p> </o:p></p><p class=MsoNormal style='text-align:justify'>After you submit your Form 471, you will receive a yellow Receipt Acknowledgement Letter (RAL). If you discover you need to make a correction on your application, many items can be corrected on the RAL.
