you are new to the E-Rate process, USAC has included a video on their website that will step you through the Form 470. To access the video, go to

option. You will need to sign the certification and mail this to USAC. I recommend using the street address listed at the bottom of the last page of the certification, and mailing in such a way that you can prove the postmark date such as USPS “Certified”
