I'm unsure how some E-rate information reaches public libraries, so I
apologize if you've seen this before.

Lorrie Germann provides instructions for making required updates to your
institutions' forms 498. As you see, for libraries, she recommends 501c3.
It does not matter that this is not technically true for most Ohio public
libraries. The important thing, she says, is that you select something. If
you're unsure, you can select "Other."

Good Morning E-Rate Community,

As many of you have discovered, USAC sent out emails yesterday (8/12/19),
to inform E-Rate applicants they must update their Form 498 to indicate a
business type as part of the “Digital Accountability and Transparency Act
of 2014 (DATA Act)”.  The Form 498 is the form filed to provide banking
information in order to received electronic disbursements of E-Rate funds.

The issue is the absence of a “business type” for public school districts,
public library systems, ESC’s, etc.

In addition, the instructions provided in the email excluded a couple
crucial steps needed to access the Form 498 and has caused some confusion.
Here are the correct steps you should follow:

   1. Log in to EPC

   2. Select the "Records" tab on the blue menu at the top of the page
   3. Scroll through the list and click on “FCC Form 498”
   4. Click the nickname of your previously submitted Form 498 and then
   "Modify FCC Form 498" in the upper right-hand corner of the screen
   5. Select the business type(s) (up to three) in order of importance from
   the drop-down menu
   6. Click "Save and Continue" to the "Further Details" page
   7. Add your company's DUN number (if not previously provided) and click
   "Save and Continue" to the certification page
   8. Review the data, click the certification boxes, and select "Certify"
   to submit the updated FCC Form 498.

As for the options to select for business type(s)  (up to three), here are
the recommendations:

*For public school districts*:

• Independent School District; and

• Nonprofit with 501C3 IRS Status (Other than an Institution of Higher
Education)

*For ESCs and Library systems (and most consortia)*

• Regional Organization; and

• Nonprofit with 501C3 IRS Status (Other than an Institution of Higher
Education)

 *For individual private schools, charter schools, and libraries:*

• Nonprofit with 501C3 IRS Status (Other than an Institution of Higher
Education)



Please note, if you are uncomfortable with choosing the recommended
options, you can choose “Other”.  While on a call with USAC, they assured
us that an incorrect choice will NOT affect your reimbursements.
