I seem to be in a picky mood this morning, so I apologize before I even get started...

Page 1 paragraph 1: "survey" omitted after "October, 2015"
  "    " : I'd suggest switching "metadata" and "technical" in the last sentence. "Metadata" can be used as both a noun and an adjective, and it's not clear at first scan which is intended -- unless it follows the "technical" adjective.

Page 1 paragraph 2: the sentence about the DPLA e-book initiative seems parenthetical, or even unnecessary, and interrupts the paragraph's logical flow.

Page 1, end: The "Ohio DPLA Steering Committee" line confused me, until I realized it was a "signature." Can we put a couple of dashes above it?

On to the questions:

#3: Wording needs fixed, a couple of editing errors there.

#4 & #5: Is it possible to have no metadata schema ("none")?

#10: Does DPLA request any particular type of thumbnail? (jpg, png, size?) Should we include that in the question?

#12: Seems like "none" is a possible answer for this one.

I’ve been working with the metadata working group on the followup survey.  I had hoped to get it out earlier this month, but that didn’t happen.  If the Steering Committee could serve as the test group that would be great.  I’m looking to make certain that the wording makes sense and that the functionality works.  Remember the folks who are taking the survey will understand the terminology, where all of you may not. Please have your comments to me by next Monday, February 2.
