[OPLINLIST] using brochures for new cards

We mail out new library cards and place a brochure with the card. We =
get paper for this brochure from paper companies that do this kind of 3 =
fold brochure. The company also has a template on a floppy in which you =
type your library information. We used to use paper with a picture of =
books on it but that paper is no longer being printed.
Does any library do that kind of brochure? Do you know of a company =
that has the paper and template that looks like a library would use?
