[OPLINLIST] Library Card Application

Hello,
We are reviewing our card application form for both adults and minors. We will start asking for birthdates for collection purposes (of legal guardian for minors), but there is so much more we could ask... I'd be interested in seeing how other libraries handle library card application/registration. If you get a moment, please fax a copy of your registration form to 419-399-2114. We are also on cargo.
