[OPLINLIST] a couple of questions about surveys

Hello all,

I would like to survey patrons about our reference collection, in order
to help us focus our goals and purpose in reference selection. Has
anyone created a survey like this, and what questions did you use?

I am also interested in surveying patrons about their adult programming
needs and interests? Has anyone created a survey like this, and what
questions did you use?
