[OPLINLIST] E-Rate Form 486 Deadlines

OPLIN and the State Library would like all public libraries to be aware
of upcoming E-Rate Form 486 deadlines for FY 2008:

 

The deadline for filing a Form 486 (containing the necessary
certifications on technology plan approval and CIPA compliance) is the
later of 120 days from either the funding date or the start of services.
The comparable Form 486 pre-Thanksgiving filing deadlines for the next
five waves are as follows:

 

                                          Wave 11
11/05/2008 (this Wednesday)

                                          Wave 12
11/12/2008

                                          Wave 13
11/13/2008

                                          Wave 14
11/19/2008

                                          Wave 15
11/21/2008

 

 

The deadline for funding awarded in FY 2008 waves 1-10, with a service
start date of July 1, 2008, was October 29, 2008.  Applicants who missed
their Form 486 deadlines will receive a reminder from the SLD shortly.
You will then be provided an additional 15 days to file the Form 486
without penalty. (Per FCC Alaska Gateway Order adopted September 14,
2006)
