[OPLINLIST] E-rate forms required for OPLIN

Every fall, OPLIN requires that every library, regional library system,
and State Library location which has an OPLIN connection submit two
forms to us for E-rate: a Letter of Agency, and a Form 479. It's time
now for this year's submission.

The procedure is simple:
1. Go to http://oplin.org/erate and enter your FSCS number.
2. Answer 10 simple questions. (We even give you last year's answers.)
3. The website will generate PDFs of both forms. Print them and sign them.
4. Fax the forms to 1-614-728-5256 and file your original. (With your
E-rate paperwork, if you apply -- and why shouldn't you?)

Please do this within the next two weeks. After November 15, we'll start
pestering any library that hasn't faxed us the forms.
