[OPLINLIST] online registration?

Hi folks!
Just a few questions.....

    * Do you allow your patron's to register online for a library card?

    * Do they have to come to the library and show proof of address
      and/or give a signature for the card?

    * Or do you mail the card?

    * Anything else we should know about your procedures?
