[OPLINLIST] Summer Reading Program Registration Forms

Hello all!

The children's librarian Arla Fry and I would like any suggestions or
feedback on how you handle registration for your children and teen summer
reading program.  We were wondering if you had a specific form you use and
how you handle getting information on medical restrictions, medical
contacts, food allergies, etc  Any sample forms sent to us would be
helpful.  We appreciate your feedback.
