[OPLINLIST] gift acceptance form/agreement

The Board has asked that I collect copies of gift acceptance agreements.
We have a policy for the donation of books and other items. And, we have
a short form we provide people acknowledging the donation. I am looking
for something a little more substantial and specific to cover larger
donations. The form would be more of an agreement between the library
and the donor. Please forward a copy on to me if your library has this
type of form. Thank you!
