[OPLINLIST] E-Rate Update and Reminders and Form 471 Workshop Rescheduled

Lorrie Germann, State E-Rate Coordinator/Program Director at eTech Ohio
has sent out the following information and reminders to the Ohio Public
Library E-Rate Community:

 

 

*         The FY2011 Form 471 application filing window will open at
noon EST on Tuesday, January 11, 2011 and close at 11:59 pm EDT on
Thursday, March 24, 2011.

 

*         Because of the opening date for the application filing window,
the Form 471 workshop has had to be rescheduled.  The new date is
February 8, 2011 from 9:30-12:00 (with registration beginning at 9:00).
The event will be live at the State Library with videoconference sites
around the state.  At this time the State Library anticipates that all
of the original sites will still be hosting the event.  If you
registered for next weeks event, you will be automatically registered
for the February date.  Registration information and locations can be
found at:  http://www.library.ohio.gov/event 

 


*         The Apply Online <http://www.sl.universalservice.org/menu.asp>
page and related tools located on USAC's website will be unavailable
starting at 5:00 pm EST Friday evening to as late as 5:00 pm EST on
Sunday. USAC will be performing its customary website maintenance to
prepare for the opening of the FY2011 filing window and also posting the
new online Form 470.


 

*         USAC has provided guidance in their weekly news brief if you
have not yet completed and certified your Form 470 using the old version
of the form.  The information can be found at

o        It's important to note Item 3:  "I have submitted a Form 470
but have not yet certified it" - After Cutover.   After the cutover, you
will not be able to certify a submitted Form 470 online.  You will need
to certify in paper form by:

*         Using your browser's Print function, print a copy of your
entire form. 

*         Write "CERTIFICATION" at the top of the first page of the
printed copy of the form. 

*         Sign and date the printed copy in Items 27 and 28,
respectively. 

*         Send the entire form to USAC at the address at the bottom of
the last page.
