Lorrie Germann, State E-Rate Coordinator/Program Director at eTech Ohio
has provided the following updates for the E-Rate program:



The new Form 470 is now available online at
http://www.sl.universalservice.org/menu.asp  .  Please be aware that
Firefox is not supported.  Large sections of the online form will not
display and several items appear in the margins.  USAC recommends the
use of  Internet Explorer 6.0 and above, as other browsers may cause
errors.  

 

Lorrie has created a step by step PowerPoint document for the new Form
470, titled "E-Rate FY2011 NEW Form 470 Step-by-Step  Schools/Districts"
and it can be found on the eTech website at
http://www.etech.ohio.gov/e-rate-technical-assistance-program/materials.
dot .  

 

The FY2011 Form 471 window opened  today at 12:00PM EST.  If you have
filed a Form 470, remember to check the Allowable Contract Date posted
on your application, to ensure you meet the mandatory minimum 28 day
waiting period before submitting a Form 471.
