[OPLINLIST] Director Evaluations

[Charlie Hansen]

The board and I are interested in beginning to do annual performance
evaluations.  This includes one for the director.  Does anyone have a sample
director's evaluation and/or self-evaluation that we could use to guide us
in creating our own process?
