[OPLINLIST] Library Surveys?

Hello Everyone,

 

Have any of you conducted a survey of your patrons to ask what they use the
library for and what services they would like to see offered? 

 

If so, could you send me a copy of your questions or what you did?

 

We are getting ready to do a survey and I'd like to see what other libraries
have done.
