[OPLINLIST] Survey: 2018 CSLP summer program manual delivery preferences (for libraries with changes only)

Hello,

The Collaborative Summer Library Program is collecting state orders for 2018 CSLP summer library program manuals for the "Libraries Rock!" theme. I'm canvassing for any changes.

The State Library will continue to use LSTA funds to purchase CSLP manuals for Ohio public libraries in any of these formats: USB flash drive (includes online access code); DVD (includes online access code); or online only. Each library system may select one format for the LSTA-supported purchase. Libraries will be able to purchase additional manuals in any format (including good old paper!) from the Upstart/CSLP catalog, available this fall.

Please take a moment from the busy-ness of SRP to complete a brief survey, ONLY IF any of these apply to you:
 *Your library has a change in contact person to receive the manuals;
 *You want a different number of manuals than you received for 2017;
 *You want a different format than you received for 2017.

If you desire no changes to your CSLP manual delivery, please disregard this survey.
