[OPLINLIST] request for attendance form

Does your library have any sort of form that a school, business, or group fills out to request staff at an event.
For example, the school would like the library to show up at literacy night...
A form may included what are the groups expectation of the library, time of arrival, length of event, do we need to bring items, etc...
