[OPLINLIST] Performance Evaluation Criteria

Hello!

If your library conducts annual performance evaluations, and if you are willing, could you please send me your library's template(s)? I'm interested in a template for any and all positions but especially interested in a performance eval template for administrative positions (managers/supervisors, director, fiscal officer, etc.).

I'm happy to compile (with permission) and send out to others who are interested in receiving templates as well.
