Do you have OPLIN-provided workstations that are no longer in use and
wish to return them to state surplus? You can send the equipment to the
State Library between March 15-29, 2004.
To return equipment, simply e-mail the OPLIN Support Center (
<mailto:support@oplin.org> support@oplin.org) with the following info:
* Write "<name of your library> Equipment Condition" in the
subject line.
* For each piece of equipment you plan to return, provide the
equipment name, the asset tag number, the serial number, and the
condition. If the condition of a piece of equipment is unusable, please
give a brief summary of the problem. If your library has in some way
altered the original configuration of the equipment, please explain.
SHIPPING
U.S. Cargo can ship computer equipment for you, but it is not part of
the statewide library materials contract.
Rates:
$36.00 for one computer set (up to three boxes)
$51.25 for two computer sets (up to six boxes)
$3.00 each additional box over 6
The computers MUST travel on a Bill of Lading for insurance coverage.
Standard coverage is $100 per shipment. Drivers can supply the Bill of
Lading at the time of pickup, or you can request blank BOLs when you
arrange for pickup.

Issue" box, indicate that this is for the transport of computers, and
provide the date the equipment will be ready for pickup.
