Jeff,
xpdf has a utility (pdftotext) to convert pdf files to text files. The
man page listed an option -layout to preserve as much as possible the
layout of the text. From their website, it looks like pdftotext will run
on windows.

"Xpdf runs under the X Window System on UNIX, VMS, and OS/2. The non-X
components (pdftops, pdftotext, etc.) also run on Win32 systems and
should run on pretty much any system with a decent C++ compiler."
