The only info I found about Bookworm (not related to the PopCap game) 
states it is an Android app that allows you to add books to lists.

You could setup a forum with phpBB (https://www.phpbb.com/). To signup a 
parent would create a forum account. Then each week create a new topic 
where parents can post their kid(s) reading numbers. At the end of the 
week or on Sunday lock the thread.

Or create a form on Google Docs which can be used for signups and 
logging. The first log submitted would act as their signup. Or create 
two separate forms. The info would be saved in a spreadsheet for 
statistics. Watch out for typos so you don't count a kid twice. Embed 
the form using an iframe or just link to the form.
