[OPLINTECH] liability waiver

With the creation of a digital technology educator position, we are going
to offer times where we will help patrons learn how to use their mobile
devices.  This position does not include resolving hardware issues.

Does anyone use a liability waiver for something similar to this?  If so,
would you mind sharing the waiver that you use?
