[OPLINLIST] Volunteer programs

Hello, all-

Our library has a number of volunteers who help us on a very informal basis.  I would guess that I am required to have some information about our volunteers on file (i.e. contact information for them should they have an accident at the library, etc.).  Does anyone have forms that they have their volunteers complete when they begin working at the library that they would be willing to share with me?  It would be greatly appreciated.  You can fax them to (419) 994-4321, e-mail them as attachments, or send them via Cargo.
