[OPLINLIST] Survey - Circulation Policies

We are revisiting some circulation policies here and would be interested
in hearing what your library is doing with the following:

1.  What do you require to register a patron under the age of 18?
 
2.  How do you handle custody considerations (joint custody, divorce,
grandparents raising the kids, etc.)?
 
3.  Do you have any restrictions on patrons under the age of 18 checking
out movies or other material?
