I apologize for duplicate postings.

The Ohio Kids Card program has been discontinued as of
Jun 9, 2005.  I called and confirmed on Aug 1, 2005
after not being able to access their website.  They
said we could recycle or dispose of all related
material.  Any questions may be referred to the Ohio
Department of Health, Help Me Grow Helpline
