[OPLINLIST] AT&T E-Rate rider form for SOMACS T1s

Attached please find the AT&T E-Rate rider for SOMACS T1 circuits. If
you buy T1 circuits through the SOMACS contract, AT&T claims this form
must be signed and dated before you submit your Form 471 (deadline for
471 is midnight tomorrow, March 24). The only purchase option available
starting July 1, 2011 will be month-to-month service (so your E-Rate 471
will be for MTM service) at $450 per month.

Here are the instructions for completing the form:

1) fill in the library name in the blank spaces at the top and bottom of
page 1, first paragraph of page 3, and in the signature box at the end
of page 5.

2) sign and date page 1 and page 5.

3) email to AF4086 at att.com. If you do not have the ability to scan and
attach a document to email, you may fax it to OPLIN at 614-728-5256 and
we will scan and email it for you.
