[OPLINLIST] Public Library Card Applications

We are looking to update our public library card application and would
like to know how other libraries protect themselves against fraudulent
cards and loss of materials.

1.       Do you have any probationary restrictions for new patrons, such
as item limits on first time check out and if a probationary period, how
long does that last?

2.       What do you use to verify address when patrons apply for a new
library card?

3.       Do you require the children to be present when applying for a
juvenile library card or, if the child is not present, do you require
other forms of identification, such as a birth certificate or Social
Security card?

4.       Are there any age specific restrictions regarding items checked
out or Internet usage (i.e. Internet usage for children under the age of
5)

5.       If the parent or guardian has a card that has been stopped due
to fines, are they allowed to get a card for a juvenile?

6.       Is there any way that you link or associate the parent/guardian
to the juvenile's card?

7.       Do you require date of birth or some type of official
government ID in order to issue a library card?

8.       Do you have a separate application for minors?  If willing to
share, please attach a copy of your library card application.
