[OPLINLIST] Tutor program

Do any libraries have a tutoring program?   If so, please send me the 
specifics of how you set up the program, guidelines, etc. Initially, I 
am considering our library being a source to obtain tutor contact names 
based on subject ...but since some of these tutors may be minors, I was 
interested to see if any of you use parental consent forms for the 
tutor's contact info to be given out, etc.....
