I thought some of you might find this information from Lorrie Germann, State E-Rate Coordinator, helpful:

In USAC's June 21, 2013 News Brief, they shared a very important "Tip of the Week":

If the status of your FCC Form 471 is "Deferred," you must contact your PIA reviewer (or call CSB at 1-888-203-8100 if you do not know the name of your reviewer) if you want to restart the review of your application. Under its summer contact procedure<http://www.usac.org/sl/applicants/step05/missing-information.aspx>, USAC will not attempt to contact applicants with a "Deferred" status application again until after September 6.

To verify the status of your application, go to http://www.slforms.universalservice.org/Form471Expert/471StatusCheck.aspx .  After clicking on "Continue" and then supplying the Billed Entity Number and Funding Year, the current status will be located in the third column in the upper right of the screen.

If you are experiencing a change in staff responsible for filing E-Rate, be sure to update the contact information with USAC.  Instructions for making a change request can be found at  http://www.usac.org/sl/applicants/before-youre-done/update-contact-info.aspx .
