[OPLINLIST] Survey: 2019 CSLP summer program manual format/delivery preferences (for libraries with changes only)

The Collaborative Summer Library Program is collecting state orders for 2019 CSLP "A Universe of Stories" summer library program manuals. I'm canvassing for any changes.

The State Library uses LSTA funds to purchase CSLP manuals for Ohio public libraries in these formats: USB flash drive (includes online access code); DVD (includes online access code); or online only.
Each library system may select one format for the LSTA-supported purchase.
Libraries will be able to purchase additional manuals in any format (including paper) from the Demco/CSLP catalog, available this fall.

Please take a moment to complete a brief survey, ONLY IF any of these apply to you:
 *Your library has a change in contact person to receive the CSLP manual;
 *You want a different number of manuals than you received for 2018;
 *You want a different format than you received for 2018;
*You want fewer or no paper Demco/CSLP catalogs (the catalog is also available online at shopcslp.com<https://shopcslp.com/>).

If you desire no changes to your CSLP manual delivery, please disregard this survey.
